Absence Form

The attached absence form must be completed to process any absences from school. The form will be used to submit any parent notes.

Please make sure to review the Student/Parent Handbook for guidelines on excused absences.

If a doctor's note will be needed, please print the form and attach the Doctor's note to this form. Keep note, that any student who has obtained 10 absences, must turn in this form with medical documentation.

Please be aware that this DOES NOT guarantee an excused absence.

ALL ABSENCE FORMS MUST BE SENT IN WITH THE STUDENT AND TURNED-IN TO THE STUDENT'S HOMEROOM TEACHER OR FISRT BLOCK (PERIOD 1 OR 2 TEACHER) UPON THEIR RE-ENTRY TO SCHOOL.

Attendance Documentation Policy

Original documentation must be submitted for all attendance-related absences, including but not limited to doctor's notes, court notices, and other official records. Photocopies, scanned copies, screenshots, or altered documents will not be accepted. Failure to provide original and proper documentation may result in the absence not being excused.
