Athletic Participation Requirements

Before a student-athlete can participate in a tryout, practice, workout, or game, the following requirements must be completed and approved:

  •  EL1  – EKG Screening Form 

  • EKG Results

    • EKG results are valid for four (4) years from the date of the screening.

  • EL2 – Physical Evaluation 

    The physical must be completed by a qualified medical professional.

    The physical is valid for one (1) calendar year from the date it is signed.

How to Submit Your Forms:

All athletic forms must be uploaded to AthleticClearance.com. Students must have all required documents uploaded and approved in Athletic Clearance before participating in any athletic activity. Hard copies of these documents will not be collected.

Insurance Requirement:

Prior to participating in interscholastic athletic competition or engaging in any practice, tryout, workout, or other physical activity associated with the student’s candidacy for an interscholastic team, each student athlete must be covered by the Athletic Insurance Fee.

NFHS Course Requirement:

All student athletes are required to annually view and complete the FREE NFHS online Concussion Course, Sudden Cardiac Arrest Course, and Heat Illness Course. Students must submit the certificate of completion for each course prior to participating in team activities.

Student Athlete Eligibility Requirements & Forms

Participation is a Privilege:

Participation in interscholastic athletics is a privilege, not a right. To maintain this privilege, students must follow established rules and meet academic and behavioral standards aligned with the values and goals of our school. These expectations are clearly outlined in our Student-Athlete Handbook, available below.

Local Rules May Be More Stringent:

Mater Bay may adopt more stringent rules for the students under their supervision. No school or school district, however, may adopt rules that are less stringent than those of the FHSAA.

Academic Requirements:

2.5 GPA Required for Academic Eligibility. A middle/junior high student must have 2.5 GPA, or the equivalent of a 2.5 GPA based on a 4.0 scale, at the conclusion of each semester. A high school student must have a cumulative 2.5 grade point average on a 4.0 unweighted scale, or its equivalent, at the conclusion of each semester to be academically eligible during the next semester
